2012 – 2013 MKP USA Certificates of Insurance

As a reminder regarding any site we use in MKP USA:
  • We need a valid use agreement or lease for any insured site
  • Sites cannot be added until we have one.
  • Each site we add costs us in additional premium, so if events can be held at already scheduled locations, they should be
  • New locations are added to a master sheet by emailing Greg Gondron, operations@mkpusa.org, using the subject:  ISSUE CERTIFICATE  in the header.
When considering using locations for our traininings and events, other than I Groups, obtain leases, contracts and agreements, then send them to me.  They will be reviewed by our Risk Management Council.  In general, when site has its own use agreement, that will save us efforts of creating one.  And, the MKP-USA agreement can be a last resort agreement between a site and MKP centers, if a site has no agreement written.  Having written agreements with sites, that fall within the bounds of our insurance coverage, brings us into compliance with our insurance coverage.

MKP USA Site Registration:

MKP USA needs to have a record of all sites that you currently use in order to get accurate quotes for our insurance policy. In order to complete these quotations for a specific initial calculation can be made, we still need to collect data on more PIT, LT, ST, other Trainings, and Council and Meeting sites.
Remember for each location, our Centers/Regions need to be in a valid agreement executed by authorized MKPUSA/MKPI and Site/Facility parties. I’ve attached a MKP USA Facility Use Agreement document for your use in this email.

MKP USA Site Use Agreement:

In order to do our work and fulfill our mission, we maintain the integrity and safety of our trainings and the sites where we conduct them. We do this by creating clear agreements and meeting our commitments. 

Please follow these instructions for the use of any Property or Facility. (hereinafter, ‘Site’)

MKP USA requires a signed agreement for use of Sites for MKP USA Center sponsored events.  These include Homecomings, PITs, Trainings, Community Gatherings, Business and Community Meetings, Workshops and NWTAs.  At times, MKP USA centers use sites that do not have their own Use Agreement, Facility Agreement, Lease or similar document. 
 
Normally, the Owner of a Site provides a document when leasing / granting / licensing / renting out a property for use.  In cases where the Owner of the Site does not have such a document, the attached Agreement can be used by an MKP USA Center.  This document has been reviewed by MKP USA’s counsel and approved by MKP USA. 

Authorized parties at a Site will decide if they want to present this document or a different agreement to an MKP USA center in connection with the use of their Site.   

If a Site elects to present this document to the MKP USA center as their Facility Use Agreement, only an authorized party or the owner of the Site may execute this Agreement.  Similarly, only a party authorized by the Center’s Board or Council may execute this Agreement for the MKP USA center.    

In the case that a Sub lessee / Tenant of a Site wishes to enter into this Agreement with an MKP USA center, the MKP USA center must confirm that the Sub lessee / Tenant has the authority to do so under their Agreement with the Site Owner prior to signing the Agreement. In such a case the cover email to MKP USA included with the Facility Use Agreement must include detailed confirmation of such authority. 

Signed Facility Use Agreements must be submitted to operations@mkpusa.org and finance@mkpusa.org 10 days in advance of any MKP USA sponsored event. Contact the Operations Director or Finance Director to request an exception before submitting  late or last minute Agreements.
